Notice of Availability of IRS Form 1095-C
Form 1095-C provides important information about the health insurance coverage offered to you by Modlogiq Inc. / Designlogiq LLC during the 2025 calendar year. This form is used to determine eligibility for premium tax credits; however, you are not required to file this statement with your personal income tax return.
How to Request Your Copy
As part of our efforts to reduce paperwork, we no longer automatically mail paper copies of Form 1095-C. If you would like to receive a copy for your records, you may request one free of charge through any of the following methods:
